What is a Home Care Package?

A Home Care Package is a program that provides Australian government-funded support to older people who want to continue living independently in their own homes. It is designed to help you with a range of care needs, from help with the housework to more complex nursing care.

You may be eligible for aged care Home Care Packages in Brisbane if you are that you are 65 years or older (50 years or older for Aboriginal or Torres Strait Islander people) and have noticed a change in what you can do or remember, been diagnosed with a medical condition or reduced mobility, had a change in your family care arrangements, or had a recent fall or hospital admission.

Eligibility is assessed through the My Aged Care portal, which is the entry point for accessing aged care services in Australia. To receive a Home Care Package, you need to undergo an ACAT assessment (Aged Care Assessment Team) to determine what aged care services best meets your needs. You can apply for an ACAT assessment online through the My Aged Care website, or by calling 1800 200 422. You can also nominate someone to apply on your behalf, if you prefer. The process takes about 20 minutes, and you’ll need to have your Medicare card handy so you can provide your details.

If you are eligible for an ACAT assessment, you will be contacted by an assessor, and they will arrange a time to visit you at home to get a better understanding of your support needs. During this time, the assessor will talk to you about your needs, what you are struggling with, and get details about any medical conditions you may have. Some things to have handy during the assessment are your Medicare card and other proof of ID, copies of any referrals you have from your doctor, and any questions you may have about aged care services.

You don’t have to do the assessment alone – you can choose a family member, friend or carer to be present with you the whole time. You can also arrange to have a translator accompany you, if English is your second language.

After your assessment, you will receive your results and whether or not you are eligible for a Home Care Package. If you are eligible for a Home Care Package, you will be placed in a national queue until a package is available to you. According to the My Aged Care website, the current wait time for a Home Care Package is around three months, although it depends on where you live and the level of care you require. Once you are allocated a Home Care Package, you can choose a provider to deliver your services in your own home.
