• Apply for an assessment through the My Aged Care portal. My Aged Care is the starting point for accessing government-funded aged care in Australia. To apply for a Home Care Package, you’ll first need to apply for an ACAT Assessment (Aged Care Assessment Team) either on the My Aged Care website or by calling 1800 200 422. The application takes around 20 minutes, and asks you questions about your health, how you’re going at home, and what services you are already receiving. You’ll need your Medicare card to do the assessment.
• Do an ACAT assessment in your home. Once you have successfully applied for an assessment,anACAT assessor will contact you to arrange a face-to-face meeting in your home. During this assessment, they may talk to you further about how you are managing with everyday tasks, ask for permission to talk to your doctor about your medical history, and explain what type of government-funded care you might be eligible for. You can have a family member or friend with you for the assessment for support.
• Receive your Home Care Package approval letter. If you have been approved for a Home Care Package, you will receive a confirming your funding, and the level you have been allocated. You are then placed in a national queue for access to home care, and be notified when a package has been assigned to you.
• Choose a home care package provider. While waiting, you can start researching Home Care Package providers who can deliver the services that are funded in your package. Only approved providers can provide Home Care Packages, as they have satisfied the legal and social requirements to do so.
