We work in care because we want people to feel supported, respected and confident in the services around them. I genuinely enjoy working with people and believe everyone deserves to feel heard, respected and supported. I bring strong communication, problem-solving and relationship-building skills to my role, combined with a practical, person-centred approach. Our approach: hands-on and relationship focused

Our Fraser Coast team is very hands-on and relationship focused. For example, when a client’s needs change, the Case Manager works directly with the family, support workers and local health professionals to adjust supports quickly rather than waiting for issues to escalate. We also place a strong focus on continuity and local knowledge. Our team knows the Fraser Coast community and works to match clients with workers who suit their needs, preferences and routines, helping clients feel more comfortable and supported at home.

“There’s something really powerful about helping someone stay in the home they love, surrounded by their memories and community. We take the time to understand each person’s needs and health journey, so we can build care that’s not only safe and practical but also uplifting. Our goal is to help people live well – and on their own terms.”

Why choose HomeCaring Fraser Coast

Care you can feel confident about.

One of the biggest concerns families have is finding care that is reliable, consistent and responsive when needs change. Families want to know that support workers will turn up, that communication will be clear, and that someone local is keeping an eye on the bigger picture.

  • Someone local keeping an eye on things

Our team responds by staying closely connected with clients, families and workers, and following up quickly when there is a change in health, routine or support needs.

  • How quickly can care start once we are ready?

We work with the client and family to understand what support is needed, complete the required onboarding steps and then begin matching suitable local workers. Timeframes can vary depending on the type of support required and worker availability, but we keep families updated throughout the process.

  • Will we have the same support workers?

Wherever possible, we aim to provide consistency and build a small team of workers who get to know the client’s routines, preferences and goals. If a regular worker is unavailable, we communicate with the client or family and work to find a suitable replacement.

  • What happens if Mum or Dad’s needs suddenly change?

Families can contact their Case Manager to discuss the change. We can review the current supports, speak with relevant health professionals where appropriate, and work with the family to adjust the care arrangement. Our focus is on responding early rather than waiting for a small issue to become a bigger one.

Local stories

Families we support on the Fraser Coast.

Support at Home, Fraser Coast

Coordinated support to stay safely at home

During a home visit, the Case Manager identified that the participant would benefit from additional support to remain safely and independently in their own home.

The Case Manager coordinated services including podiatry, physiotherapy, continence support and regular in-home domestic assistance, and worked closely with the participant’s family to make sure their changing needs were identified and the right support was put in place.

These coordinated services have supported the participant’s ongoing independence, wellbeing and ability to remain at home longer, all while being mindful of the quarterly budget and working within the guidelines of the Support at Home program.

Support at Home, Fraser Coast

Support after surgery

Following recent surgery, the participant needed more in-home services and access to allied health supports to help their recovery and ongoing wellbeing.

The locally based Case Manager worked directly with the participant to identify her individual needs and coordinate the right services and supports.

By keeping in regular contact and involving the participant in decisions about her care, the Case Manager helped create a safe and supportive home environment while promoting the participant’s independence, choice and control.

Part of the community

Our team and local connections.

  • Support beyond the main centres

Travel distance, worker availability and continuity can make support harder to find in smaller Fraser Coast communities. Our team addresses this by building local worker networks, planning support carefully around each area and working closely with families and local health professionals to create practical, sustainable care arrangements.

  • Seniors in Focus

The Halcro Street Community Centre runs its Seniors in Focus program, which provides information, referrals and activities for older residents.

  • Local resources for older people and families

Fraser Coast has a strong network of community resources for older people and families. Useful local resources include Fraser Coast Regional Council’s seniors services and community directory, the Hervey Bay and Maryborough neighbourhood centres, and local dementia-focused initiatives.

  • Dementia-friendly communities

The Fraser Coast Dementia Alliance supports awareness of dementia-friendly communities, and Council also promotes activities such as Seniors Month and the Over 50’s Lifestyle Expo. These help our team stay connected with what is happening locally and help families find relevant community supports.
