Out of Hospital Care (OHC) Program

The NSW Health Out of Hospital Care Program helps patients get back on their feet after they have been discharged from a public hospital. It provides assistance with things like housework, getting dressed, travelling to appointments and cooking meals, so you can focus on your recovery.

Home Caring provides services under the OHC Program under three different packages:

  • Community Packages (ComPacks)
  • Safe and Supported at Home (SASH)
  • End of Life (EoL)

Our support workers and case managers provide high quality care that is tailored to your unique needs. We’ll help you with everyday tasks, so you can transition well from hospital and resume living independently at home. You can also transition between packages, depending on your changing needs.

Community Packages (ComPaks)

This is a short-term package of services that helps you regain independence after you’ve been in hospital. Available to participating NSW public hospitals, it can help you have a safe and early discharge, so you can get back to everyday life as soon as possible.

ComPaks includes services such as:

  • Personal care such as help with bathing, hygiene and getting dressed
  • Domestic assistance to help with cleaning, shopping and laundry
  • Meal preparation or meal deliveries to your home
  • Transport and social support, such as help with the shopping
  • In-home respite to give carers a break

In some areas, ComPacks can be used as an early intervention to avoid a stay in hospital. This is called ComPacks Healthy at Home. To find out whether this is offered in your area, please get in touch.

SASH (Safe and Supported at Home)

This program supports people with a functional impairment who are in the process of applying from the NDIS, or are ineligible for NDIS funding. We’ll help you with daily living tasks, so you feel supported in your own home.

SASH is available for six weeks, but can be repeated depending on your needs. Our services include:

  • Personal care such as help with bathing, hygiene and getting dressed
  • Domestic assistance to help with cleaning, shopping and laundry
  • Meal preparation or meal deliveries to your home
  • Transport and social support, such as help with the shopping
  • In-home respite to give carers a break 

End of Life

This package is designed for those who are in the terminal phase of their illness, and who wish to die in their own home or live at home for as long as possible. We also provide support to family members, taking care of everyday tasks so you can spend quality time with your loved one.

EoL packages are available for six weeks, but it can be repeated depended on your needs. It includes:

  • Personal care such as help with bathing, hygiene and getting dressed
  • Domestic assistance to help with cleaning, shopping and laundry
  • Meal preparation or meal deliveries to your home
  • Transport and social support, such as help with the shopping
  • In-home respite to give carers a break
