Lara Furletti, Director, Registered Nurse and Case Manager

A Registered Nurse with over 15 years’ experience, Lara has worked in aged care facilities, support homes and hospital settings. She is currently a Clinical Nurse Specialist in an operating theatre. Lara is passionate about helping people improve their quality of life, no matter their age or ability. She cares for her son who is living with a disability, and knows the difference high quality support can make to families’ lives.

Rey Garcia, Director and Registered Nurse

Rey is a Registered Nurse with experience in aged care, operating theatres and hospital , as well as six years’ experience as a paramedic in Kuwait. He is adept at working with multidisciplinary teams to deliver the best outcomes for patients. Rey believes supporting someone with a disability, or the older members of our community, is the most rewarding career you can have. He loves building people up, so they can reach their goals.

Don Fontabla, Case Manager

Don is a capable and caring care professional who is passionate about helping his clients meet their needs, reach their goals, and make the most of their government funding. He has worked in client-facing, management and administrative roles in the aged care sector, and has strong communication and problem solving skills that he uses to meet his client’s needs. Don works collaboratively with his clients and their support network, helping everyone achieve the best for optimal care.

Lester Fernandez, Director and Case Manager

Lester Fernandez has worked in Allied Health for 12 years as a Theatre Technician. On any given day, he would tend to urgent patient needs, share comforting words before surgery, manage pressure points, or address positioning so patients are more comfortable. Lester provides his services with passion and dedication. He treats his clients with care and respect – as if they are part of his family.

NDIS and Aged Care

 Home Caring Vermont is a registered NDIS and Home Care Package service provider. If you are at the beginning of your support journey, we can help you apply for government funding and guide you through the whole process.

We take a person-centred approach to everything we do, which means our care is flexible to your schedule. You’ll also have peace of mind as all of our carers are locally based, rigorously screened and dedicated to providing compassionate support.

Whatever you need – whether it’s help with daily tasks, staying in touch with the community, transport to appointments, or staying safe at home – we’ve got you covered.

NDIS Support Coordination

Navigating the NDIS can be an overwhelming process. Our Support Coordination service makes it easy for you, so you get the right support for your needs. As health care professionals, we are experts in clinical and disability care. You can be confident that we understand what works best, as we partner with you in achieving the kind of life you want.

Our NDIS Support Coordinator can help you:

  • Understand your approved supports and make informed decisions
  • Coordinate a range of support service that meet your goals – we’ll be with you every step of the way, while also empowering you to take charge of your support
  • Source quotes and service agreements from providers
  • Monitor and review individual budgets, service agreements, and how you’re progressing towards your goals
  • Learn how to access and use the NDIS online portal, so you have direct access to your funding
  • Prepare for NDIS meetings
  • Include family members and loved ones in the coordination of your support, if required.

TAC and WorkCover services

Your recovery is the most important thing after a road accident or workplace injury. We’re here to offer support along the way.

Home Caring Vermont is a registered provider with the Transport Accident Commission and WorkSafe in Victoria. We can help you recover at home after surgery due to an accident; provide support for permanent injuries, or give you a hand with daily tasks while you get back on your feet.

Our TAC and WorkCover services include:

  • attendant care support such as personal care, transferring in and out of bed, helping you settle back home after a stay in hospital
  • home services such as housework, laundry, and preparing meals. We provide support until you are able to take on tasks for yourself
  • supported accommodation in a safe and home-like environment with all the support you need
  • Disability support services to help you build your skills, engage with the community and increase independence
  • independent living aids and specialised equipment such as wheelchairs, walking frames, hospital beds and lift
  • home modifications such as installing hand rails in the bathroom or an access ramp to your home;
  • 24-hour care to help you live safely at home during the recovery process
  • child care services to help you care for your children until you get back on your feet
