Our transition care provides elderly patients with the short term support they need after they have experienced a stay in hospital. As well as being time-limited,  this is therapy focused and built around set goals. We help elderly patients recover, improving their physical and mental health over a short to extended period.

Packages available may include low-intensity therapy as well as social work, personal care and nursing support. Our aim is to ensure that every client can gain the professional support they need to improve their independence as well as their functioning.

The care is provided at home, providing your loved one with the best environment for recovery. At Home Caring, we firmly believe in the benefits of individuals being able to stay in a home that they love and still receiving the support that they need.

With the right level of support, we believe that entry into a residential aged care facility can be effectively delayed. In doing so, elderly patients will be able to spend more time in their home where they feel comfortable, independent and secure.

About Transitional Care 

Aside from ensuring that entry into a full care facility can be delayed temporarily or indefinitely, for elderly patients, there are various other aims of transitional care. For instance, we strive to ensure that patients do not have to stay in the hospital for an extended period. Instead, they can continue to get the treatment and support they need in the comfort of their own home.

Patients are always assessed before they enter a period of Transitional Care. If a doctor has said that a patient is ready to be discharged, then they will be able to gain the benefits of the transitional care program that we provide. We can even continue nursing treatment to ensure that there is a smooth process in moving from the hospital to the home.

Usually, the target time for Transitional Care will be 12 weeks. However, this can change depending on the patient and their individual needs. We are fully prepared to respond to special requirements and the particular needs of an individual loved one. Our aim is to ensure that each individual gets the full level of support that they need.

When Will Transitional Care Be Provided? 

Transitional Care will typically be provided to a patient after they have experienced an acute episode which would have required hospital admission. The multidisciplinary transitional service would typically include helping ensure that patients can gain low intensive therapy as well as full nursing care.

As well as helping to improve an individual’s physical independence, we also strive to ensure that we can help increase their confidence as well. By working to develop the confidence of the patient, it is often possible to return and live safely in their home once more. In this way, we ensure that we do focus on your loved one’s physical and mental wellbeing. We can provide the right, customised support to guarantee they have the building blocks in place that they need to improve and regain their previous standard of health before their stay in hospital.

What Does Transitional Care Involve?

If a patient is ready for transitional care, they will return home. However, they will still gain complete support from care providers. As transitional care providers, we will deliver an assessed care plan and offer complete support from health professionals.

As noted, the care plan will typically consist of a total of 12 weeks. However, in some cases, the plan can be extended. This will be the case if a patient does require additional levels of support and an extra level of therapeutic care.

Transitional care will always be designed and based on the needs of the individual patient. As such, there can be numerous different parts of a transitional care package. You will be able to ask any questions that you may have and we will strive to ensure that all your concerns are handled with the greatest respect.

Typically, the main form of support during transitional care will be low-intensity therapy. However, other options are available, again depending on individual needs.

What Does Low-Intensity Therapy Mean?

Low-intensity therapy is the typical support provided through this care option. There are numerous possibilities that might be relevant to you depending on your needs and requirements. This includes physiotherapy and podiatry. With this option, it is possible to help increase mobility which will be a vital part of ensuring that an individual is able to regain their independence. Low-intensity therapy is suitable for the home and will suit people with a range of different requirements. For instance, low-intensity therapy will provide benefits for anyone who is recovering from a long term injury. It can also help an injury get worse. This can happen, particularly if your loved one was in the hospital for an extended period.

Nursing support and clinical care may also be offered depending on the needs of the patient. After a stay in the hospital, it’s common for a patient to need nursing support to help them recover from an acute medical condition.

A patient may also be provided with access to a social worker as well. A social worker will be able to help an individual tackle both emotional and physical issues that may be blocking them from achieving a higher level of independence. At Home Caring we provide the full support and solution that your love needs.

Some examples of the care available include stretch to improve movement and mobility. Alternatively, some clients may not support with personal care such as bathing and washing. They may also require assistance with their meals. As mentioned, these packages are completely customisable. So, you will always be able to find the options that you need using our services.

We can also change the solution offered overtime to ensure that the package continues to match the needs of the patient as they improve over the period of transition. We will also monitor their progress and advise whether your loved one may require an extended level of support. Our aim will always be to provide the best option possible to your loved one. The Australian Government has announced significant changes to the aged care system introducing the Support at Home program set to commence on 1 July 2025. This initiative aims to enhance support for older Australians enabling them to remain in their homes longer and receive more personalised care.

Key Features of the Support at Home Program:

• Reduced Wait Times: The program will shorten the waiting period for in-home aged care services ensuring timely assistance for those in need.

• Tailored Support Levels: Support at Home introduces eight levels of ongoing care replacing the previous four, to better match individual needs.

• No Caps on Cleaning and Gardening: Unlike previous programs there will be no annual caps on cleaning and gardening services allowing for more comprehensive support.

• Care Management Cap: A 10% cap on care management costs will remain in place to maintain affordability.

• Assistive Technology Access: The program will expedite access to assistive devices like walkers and wheelchairs enhancing mobility and independence.

• Increased Support Funding: The maximum support level will rise from $61,000 to $78,000, providing more resources for those with complex needs.

• Restorative and Allied Health Services: There will be a temporary funding boost for restorative and allied health services, aiding recovery and well-being.

• Palliative Care Support: The program includes provisions for palliative care allowing older Australians to spend their final weeks at home surrounded by loved ones.

Staged Implementation of Price Caps:

Price caps on services will be phased in starting from 1 July 2026. For the first year providers will continue to set their own prices similar to the current Home Care Packages program. In early 2025, consultations will be held with older people, advocates, families, carers and the sector to ensure fair pricing and robust consumer protections during this transition.

No Worse Off Principle:

To protect existing Home Care Package recipients, the government has introduced the “No Worse Off” principle:

• Grandfathering: Individuals approved for a Home Care Package as of 12 September 2024 will transition to the new system without financial disadvantage.

• Fee Structure: If you were a full-rate pensioner paying no fees under your Home Care Package as of 12 September 2024, you will continue to pay no fees under Support at Home. If you were required to pay fees, you will pay the same or less under the new program.
